Medication Pick-up
If your student has medicine stored in the school nurse’s office, it is time to pick it up. All over-the-counter medication will be sent home with the students on Tuesday, May 26th. If you do not want your child transporting his/her medication, please come by the Smithfield Middle nurse’s office to pick it up before then. Narcotics must be picked up by an adult. Any medicine not picked up before the summer break will be discarded. If your student will require more than 10 doses of a medication next year, please have the physician fill out a medication permit during a checkup this summer. If you wish for your child to carry his/her own inhaler next year, please have the physician sign a medication permit in order for this to be allowed.Other important things to remember:
All 8th graders must have an up to date Tetanus booster or they will not be allowed to register at the high school.
All 7th graders must meet the new immunization requirements (listed below) before they will be allowed to register at Smithfield Middle School:
(Required for entry to 7th grade if at least five years have passed since the last dose of tetanus).
2. Meningococcal (Menactra)
(One dose required for entry to 7th grade).
3. Varicella (Chickenpox)
(Two doses required for entry to 7th grade unless had disease).
